This is the first time in all my heli flying days that I forgot to Locktite something as critical as the grub screw on the tail hub. The grub screw was tight and checked.
This happened on a third flight. First was a runup to check tracking , first and second flight for trimming and then third flight doing lazy patterns lost tail control, started spinning and in my haste to get it to the ground I tore up the fuse pretty good and trashed the head.
Granted, it wasn't anywhere near the caliber of detail and precision work that others here are doing, but still a dumb mistake . I have already started glassing it back together. I also still need windscreens and now blade grips, spindle, main shaft and flybar.

Not the wrong direction. Have a look at the hub compared to the tail shaft.

That stinks. How i lost my 76. 26 1/2 flights and a grub screw backed up.
